The church was built with a dedication to the Virgin Mary between 1158 and 1183, when the Ezzelini family ruled in Bassano del Grappa. According to ancient tradition, it was built at the behest of Ezzelino II il Balbo, a veteran of the Holy Land, to fulfill a vow.

Like many churches of the time, it was built at the gates of the city to welcome pilgrims arriving after long, tiring and dangerous journeys. Between 1287 and 1292 it was handed over to the Friars Minor, who dedicated it to Saint Francis.

The building is in Romanesque-Gothic style with a Latin cross layout with stones and pebbles from the River Brenta, alternating with strips of brick; the facade has a gable structure with an agile prothyrum with round arches from 1306.

The lunette contains frescoes of the Madonna, the Child, Saint Anthony and Saint Francis, the work of Luca Martinelli in 1613. To the right of the prothyrum is a special representation of the Annunciation depicted according to the Valentinian heresy with the already formed Child who of the Holy Spirit into the womb of Mary, created by Battista da Vicenza in the first decades of the 15th century.
