The arduous way over the many stairs is worthwhile. I would recommend a visit in the morning or in the evening.
With the entrance ticket for the city wall you can also visit the fort. Here some scenes were shot for GoT. When we were there, there was a GoT guide.
This made the rather bald fortress much more interesting.
On another trip to Dubrovnik, I would probably choose a guide.
