The Mittelradde rises a few kilometers east of the hilly Geest landscape of the Hümmling. Its source is northeast of Bockholte, a district of Werlte, and west-southwest of the main town of Vrees, north-northwest of the Flaschenmoor, which is largely designated as a Bockholter Dose nature reserve - near the Hambrink (37.5 m) and Varel (38 m) elevations.

The Mittelradde flows in a predominantly south-westerly direction roughly parallel to Nordradde in the north-west and Südradde in the south-east. Initially, it runs east of Bockholte southwards through the Flaschenmoor. There it turns to the southwest and passes Werlte, Auen and Holthaus. The river then runs past Wehm, Wieste and Wachtum as well as Vinnen, Lahn and Ahmsen. It then feeds the Hüvener Mühle near Hüven on Landesstraße 65 and passes Lähden, Groß Berßen, Klein Berßen and Westerloh. The Mittelradde then flows past Hulsen and Lohe and - after crossing under the Meppen-Haselünner railway and the federal highway 402 - to Lahre.

After passing Klein Dörgen, a western district of Haselünne, the Mittelradde then flows into the Ems tributary Hase, which comes from the south-east.
