    The building, which is essentially Gothic, has the form of a three-aisled hall church with a transept and polygonal apse. The floor plan and structure are based on the mendicant church of San Domenico in Perugia, which is around a century older but has not been preserved in its original form.

    The largely unadorned façade faces Piazza Danti in the east, while the actual front is on the southern side of the nave. Here a wide staircase leads to the side entrance, where the bronze monument of the enthroned Pope Julius III (Vincenzo Danti, 1555) stands and to the right of it a small pulpit protrudes from the wall, which was put together from older parts and on which the Franciscan saint Bernardine preached to the Sienese in 1425. To the side of the baroque portal from 1568 you can see the beginnings of the unfinished marble cladding, which is patterned like a carpet with Gothic quatrefoils and was stolen from the cathedral in Arezzo in 1335. Further to the left are the four arcades of a loggia from 1423.

    The rich furnishings of the hall church are predominantly from the late Renaissance and early Baroque periods, the picture windows from the 16th and 19th centuries, and the high vaults were painted in the late 18th century. The first chapel in the right aisle is dedicated to St. Bernardine. It contains the Deposition from the Cross by Federico Barocci (1569), one of his major works, which with its soft color transitions already foreshadows the painting culture of the 17th century. In 1565, the Flemish painter Arrigo Fiammingo designed the stained glass window with Bernardine's sermon. A large marble facade from 1477 is placed in front of the baptismal chapel. On the right side is the Sacrament Chapel with an altarpiece on the theme of Pentecost by Cesare Nebbia (1536–1614). Behind the wrought iron grille in the first aisle bay on the left is the Chapel of St. Joseph, in which the legendary wedding ring of the Mother of God is venerated as a holy ring in an elaborately gilded tabernacle. Also noteworthy are the paintings Martyrdom of St. Sebastian by Orazio Alfani (1576), Stoning of Stephen by Giovanni Baglione (1608), and Our Lady Surrounded by Saints by Giovanni Antonio Scaramuccia (1616). Of particular religious importance is the Madonna delle Grazie by Giannicola di Paolo on the third pillar on the right. The choir stalls from 1486-1491 with their outstanding inlays were damaged by fire in 1985. To the right of the presbytery is the sacristy, the frescoes there are from the 1570s.
