Built in 1500 to mark the transfer of the bishop's seat from San Leo to Pennabilli, the Cathedral dominates Piazza Vittorio Emanuele overlooked by the Palazzo della Ragione, built by the Medici Lordship, with the Fountain of Peace in the center, built at the point where the deed of union of the Castles of Penna and Billi was signed.

In neoclassical style, it underwent a massive restoration between 1910 and the next 30 years. They are new: the flooring, the sacristy, the access to the altar, the choir, the side chapels, the facade of the bell tower and the main door.
