The brick building, whose foundation consists of boulders, was built around 1455. The wooden tower was only built later, because according to the strict rules of the Cistercian monks, many churches were built without towers. The St. Catherine's altar, created around 1480, has been preserved from the medieval period and probably only came from Stralsund to Middelhagen after the Thirty Years' War. Another very valuable piece is a chalice made of silver and gilded in 1597.

The brick church of St. Catherine in Middelhagen was built in 1455 in the Gothic style on a foundation of field stones. At that time, the Mönchgut in the southeast of Rügen ("dat Mönnike Guedt") still belonged to the Eldena monastery near Greifswald. Particularly worthy of mention is the medieval St. Catherine's altar, built around 1480, which probably came to Middelhagen from the Hanseatic city of Stralsund after the Thirty Years' War. At the time, the Mönchgut was a pledge of Stralsund. The roof turret, the large stair tower and the vault of the nave as well as the benches, the pulpit and the well-preserved offertory box were built around 1610. The church in Middelhagen has been portrayed by painters and illustrators such as Adolph von Menzel and Lyonel Feininger.

Also worth mentioning is the church's chalice, which is made of silver and gilded and dates from 1597. In addition to the St. Catherine's altar, the parish church also contains the votive ship "Perth", which eight Rügen pilots donated to the church in 1842. According to tradition, Albert Schweitzer even once played on the Grüneberg organ, which was built in 1862. Since the cup of the baptismal font was reworked in the 19th century, its exact dating is hardly possible. However, it most likely comes from a church in the fishing village of Reddevitz that no longer exists, where it could have served as a holy water font.
