The building was erected in the years 1936-1938 (according to the register of monuments in 1930) for the purpose of serving as a tomb of the Lutomski family, the then owners of Grzybów. Currently, it serves as a cemetery chapel.

The walls of the chapel were built from fieldstone blocks, multi-coloured granite. The shape of the chapel imitates the forms of the early Romanesque rotunda of the Blessed Virgin Mary (Saints Felix and Adauctus), built at the turn of the 10th and 11th centuries on Wawel in Kraków. Built on a tetraconch plan, i.e. four semicircular apses adjoin the central rotunda. Covered with a roof with roof tiles, a monk and a nun. There is a small crypt under the chapel.
