Klein-Gelmen originally belonged to the parish of Gelinden and as such the church was a quarta capella of that of Gelinden.
In the year 1000, according to archival sources, there was already a chapel dedicated to Our Lady and Saint Eligius. The chapel and the mother church then belonged to the Sint-Maternus chapter of Liège. One third of the tithe belonged to the Sint-Maternus chapter, two thirds belonged to the provost of the Sint-Lambertus cathedral in Liège.
In 1560 the tower was rebuilt or built.
In 1796 Klein-Gelmen was separated from the parish of Gelinden and was added to the parish of Heers.
In 1835 Gelinden became an independent parish.
In 1879 the old chapel was demolished and then in 1881 a new church was built. This was done according to the design of I. Gérard, whereby the church was situated in the same place but moved slightly more to the west.
