The chapel of Our Lady located on Anna Marii Street in Otrębusy was consecrated on September 7, 2003. The ceremonial consecration of the original chapel was performed by Father Prelate Zdzisław Peszkowski together with the then parish priest of the church in Otrębusy, Father Tadeusz Aleksandrowicz. The main founders of the statue of the Blessed Virgin Mary are Dr. Jacek Zawadzki and Dr. Marek Dunin.
