The Bilsbek is a right-hand stream of the Pinnau. It rises in the Himmelmoor between Hemdingen and Quickborn and flows through the communities of Ellerhoop, Borstel-Hohenraden, Tornesch and Prisdorf. The Bilsbek lowlands are an important natural area, although parts of the Bilsbek were regulated in earlier years. There are still wet meadows and floodplains on the edge of the Bilsbek. The Bilsbek flows into the Pinnau at river kilometer 15.4. The Bilsbek forms the border between the municipality of Prisdorf and the town of Tornesch on its lower reaches, and flows into the Pinnau at the three-municipality corner of these municipalities with the municipality of Appen.

There is a Pinnau-Bilsbek-Gronau water association, a Bilsbek riding club and the MSC Bilsbek.
