    The Catholic chapel of St. Sebastian in Holzheim, a municipality in the district of Dillingen on the Danube in the Bavarian administrative region of Swabia, was built in 1716 on the Sebastiansberg, southeast of the town. In 2008 the chapel was again given a copper onion dome after its original baroque dome was replaced by a pointed helmet in 1875.

      The 25 meter high tower rises in the northern choir corner, the square, four-story substructure of which supports a two-story, octagonal structure with an onion dome. Blind fields with slit-like openings are cut into the substructure on all four sides and large arched niches with coupled sound arcades on the fourth floor. The tower octagon is divided by corner pilaster strips, blind panels and rectangular windows drawn in at the top and bottom.

      The west facade is crowned by a baroque volute gable, which is capped by a segmental arch. In the niche of the gable field there was probably once a sculpture of St. Sebastian, the chapel's patron saint. The outer walls of the nave and choir are structured by buttresses. The single-aisled nave is pierced by transverse oval windows and, like the choir, is covered by a basket-arched barrel with stitch caps. The retracted, semicircular closed choir has highly oval openings. The western end of the nave is formed by a gallery resting on pillars.
