There is no precise information on its origins. It is certain that in 1181, Gregory Bishop of Forlimpopoli granted this church called Madonna del Lago to the Camaldolese monks of Urano.

The church seems to have risen near a stream, according to legend, some religious were able to snatch from the hands of the iconoclasts of Constantinople the miraculous image of the Virgin, who must have been prey to the flames.

The cult and devotion of the Bertinoresi for this image, painted with a gold background on canvas applied to the table, is still today one of the most cherished relics in religious memory.

In 1279 the sanctuary was enlarged and consecrated, from that year the feast of the Madonna is celebrated on the Sunday after the Ascension with a solemn procession to the cathedral of Bertinoro.
