Císařská Cave is located in Ostrovský žleb on the northeastern edge of Ostrov u Macochy in the Moravian Karst. It was therefore sometimes referred to as the "Island Water Cave." The cave was discovered by Johannes Anton Nagel, who conducted the first survey in 1748. The old count Hugo František Salm, who introduced Emperor Francis I and Empress Maria Theresa in 1804, was also interested in the research.

The cave was discovered as early as 1748. The name was changed after the visit of Emperor Francis I in 1804. The name Císařská Cave has remained to this day. The cave was opened to the public in 1930. In 1952, however, it was closed to the public. The length of the corridors is 650 meters. The main corridors are formed by karst lakes with branches. In 1997, a treatment center for children undergoing speleotherapy was built here. Once a year, a religious pilgrimage takes place here.
