As reported on Wikipedia, the first apparition of the Virgin took place in April 1001, to a count of Ariano and a shepherd in the Cervaro wood.
Near the tree where the apparition took place, the shepherd filled a "caldarella" with a "mesata" of oil and made a votive lamp which still today, out of popular devotion, continues to burn inside the sanctuary.
