Cared for by the Consolata Missionaries since 1925, the Sanctuary was built in 1603 around an ancient 16th-century tabernacle whose niche features a fresco of the Madonna and Child with Saints by an unknown artist. The church had various titles: of the Assumption, of the Annunciation, of the Consoler of the Afflicted, but the population continued to call it Madonna del Monte after the hill on which it stands.
The small Baroque-style sanctuary still retains intact, despite damage from the last two wars, the original secluded atmosphere of the hermits who guarded it until 1787. The church was then closed, but the special popular devotion for the sanctuary led to its forced reopening in March 1790. In 1829, the church and its appurtenances were purchased by Giovanni Battista de Tacchi, who reopened it. The harmonious Baroque-Rococo façade is the work of Giovanni Scottini di Lizzana (1751). The votive offerings are numerous and interesting. The high altar, enclosed by a railing, is of great artistic importance.
