Done clockwise. It is not impossible as someone else said, only quite tiring with an average gradient above 9% and tears on 12%. The coastal part downhill at sunset is one of the most beautiful things I've ever done on a bike.

From Porto S. Stefano you go up quite steeply, then it becomes less demanding once you arrive at the crossroads for La Riccia.
Spectacular views of Giglio and the western coves.
The descent towards the town is very steep and requires a lot of concentration.
