The construction of the original sacred building, by the vicinie of Lera and Fontana, probably dates back to the early 16th century, in fulfillment of a vow made with San Rocco following an epidemic (some sources report the original dedication to San Sebastiano and San Floriano). Morizzo dates the erection more precisely to 1502. The Veronese pastoral visit of 1549 testifies to it still being incomplete: on that occasion its demolition or the immediate termination of the works was ordered. Following the plague epidemic of 1630, the building was expanded at the expense of the vicinie of Lera and Fontana; the presence of the decorative apparatus with heads of cherubs of 16th century workmanship in the presbytery leads to the assumption that the expansion affected the church as a whole with the exception of the latter. It is possible to refer to this campaign of interventions the endowment, towards the middle of the century, of the two altars of Castionese workmanship dedicated to Saint Anthony of Padua and to the Madonna Addolorata attributed to Giacomo Benedetti, and of a third, also Castionese, dating back to 1684; in addition to the altars, a stone clipeus inserted in the lock of the vault of the presbytery depicting Christ Pantocrator attributed to Guglielmo de Manueli of Avio also dates back to the period in question. The territory of the parish of Brentonico remained subject to the diocese of Verona until 1786, when, by consistorial decree, it was united to the diocese of Trento. The historian Silvino Pilati reports that in the years 1806, 1809, and 1848 the church was converted into a military hospital for the use of German and French soldiers.
