Located in the town enclosure, it is dedicated to Our Lady of the Assumption, to Saint Sebastian and to Saint Rocco. It is only open for the celebrations of the owners, in which a mass is sung and the Holy Office of the Madonna is recited. It is poorly furnished, but this little is in good condition. It has no dowry, no income, it lives on alms and is administered by a Prior elected every three years by the population in accordance with the Synodal Regulation (Don Angelo Cassini).
There is no precise information about the date of its construction, which occurred in fulfillment of a vow, for the town having been spared from a plague (17th century). It was rebuilt, towards the end of the 19th century, based on a design by Count Ferrari.
Inside there is a painting, dedicated to Saint Sebastian, made by the painter and current mayor of Orsara, Beppe Ricci.
