From the parish church of Pognana Lario, a mule track leads to the church of San Rocco; this mule track is flanked by chapels of the Via Crucis, decorated with mosaics, built during the twentieth century and ends with an arch which leads to the churchyard in a panoramic position.

The church has a facade built in Baroque style during the eighteenth century: it is the only external part plastered, the rest is in exposed stones; the bell tower rises along the right side.

Its origin is not known exactly; it certainly already existed in the 14th century. It is assumed that the religious building was built in place of a previous oratory; it reached its current appearance through modifications and expansions that followed one another over the centuries and which reached their peak during the Baroque era.

The church of San Rocco was the parish church of Pognana Lario until 1744, the year in which the new parish dedicated to the Holy Trinity was consecrated; with the foundation of the new church, the old one was "downgraded", lost its dedication to the Holy Trinity and was dedicated to San Rocco.
