The cathedral, which was completed in the 14th century, was rebuilt several times in the following centuries. The facade, which was built in the 13th century, was given its current shape in the 16th century.

The Cathedral stands on the site of a Roman structure on the former Roman Forum, at the northern end of what is now Piazza del Popolo in the center of the town of Todi. The earlier church, believed to have been built around the year 1000, was almost completely destroyed by a fire in 1190, while significant alterations were made after the 1246 earthquake. The rebuilt cathedral was completed in the 14th century, but the structure has been renovated and altered several times since then. The facade by Lorenzo Maitani dates from the 13th century, but has been modified several times, most recently in the 16th century
