St. Maternus is considered the patron saint against fever, contagious diseases, and the prosperity of wine. This latter fact explains why he is especially venerated in wine-growing regions. The Maternus Portal of Cologne Cathedral tells the legend of the saint: Maternus was sent to Gaul by Peter along with Eucharius of Trier, Valerius of Trier, and Clement of Metz. Upon arriving in Avolsheim in Alsace, Maternus is said to have collapsed from exhaustion, whereupon his companions became discouraged and returned to Rome. Peter, however, gave them his shepherd's crook, with which they traveled back to Alsace. With the help of this crook, they revived Maternus, who had already been in his grave for six weeks. The church was built in the 12th century after the construction of Are Castle around 1100 by the Counts of Are as a Romanesque basilica and was first mentioned in documents in 1166. In place of an earlier, higher apse, the Gothic choir was added to the Romanesque building at the beginning of the 14th century and was inaugurated in May 1326. In the 15th century, the central nave was fitted with late Gothic ribs growing out of the cross vault, and the previously installed flat ceiling was replaced. The entire cross vault of the transept and side aisles also dates from this period. The current Fatima Chapel was originally built as a sacristy around 1500. After a difficult period, the Peace of Rastatt in 1714 brought peace to Altenahr and time to repair the church and add uniform baroque elements. The church suffered severe damage during the nine-month siege of the castle in 1689/1690 and the burning of the town on January 9, 1690, which is why the visitation reports from this time describe it as being in very poor condition or no longer exist at all. At the end of the 19th century, the church was expanded. The southern transept was added with a large open staircase and the northern tower, and the nave was extended in 1893. It was not until 1892 and 1893 that the windows of the transepts were given their current shape; until then they were smaller and higher. Originally, the church had a west portal and two smaller entrances on the north and south sides. In 1927, the sacristy, which had been added in the 16th century, was opened to the church. Today, this memorial chapel, set up in honor of those who died in the First World War, is designed as a Fatima Chapel. In 1934, security work was carried out on the main nave and crossing. From 1950 to 1960, war-related damage was repaired. In 1978 and 1979, more extensive work was carried out on the church, including drying out the outer walls and renewing the floor and heating. After completion of the Altenahr bypass in 1998, the drive through the Ahr Valley was to be placed under God's protection. For this purpose, the statue "Merciful Jesus" was set up in the "Sattelfeld" corridor above the practice tunnel. The idea for this came from Maternus Gasper. The district administration and the road construction authority in Cochem supported the initiative.

The chapel was designed by Oberbaurat a. D. Pelegrini from Hanover built. K. P. Bohr from Trier designed the interior design. The chapel was consecrated on October 24, 1962. The chapel has an altar table made of Ettringer tuff. A contour mosaic of St. Maternus adorns the wall to the left behind the altar table. A church window in the chancel shows the symbols of the four evangelists. Four restored wooden figures from the 18th century adorn the chapel: St Maternus, Maria, St Joseph and St Anthony of Padua. The figures had stood in the old chapel until 1944. The wooden figure of the chapel patron comes from the former Maternus chapel on the Gieretsberg. The church windows were made in 1998 by the art glassworks Jürgen Maur from Ahrweiler. In July 2021, the chapel was badly hit by the 2021 Western and Central Europe floods.

The patronage of "Maria (or in another spelling: Mariä) Verkündigung" of this church in Altenahr refers to one of the many memorial or feast days in honor of the Mother of God Mary. The Feast of the Annunciation to Mary (liturgical name: Annunciation of the Lord) is celebrated on March 25th, exactly 9 months before Christmas. It refers to the message that the angel of the Lord brought her that she should give birth to the Son of God - Jesus Christ. According to the report in the 1st chapter of the Gospel of Luke, this happened in Nazareth (Israel). There, on the spot where this happened - according to a tradition dating back around two thousand years - there is a large church, which can therefore be considered the 'original patron church' for the title of this church in Altenahr.

The Catholic parish church of the Annunciation of Mary is a listed church in Altenahr, Rhineland-Palatinate, a local community in the Ahrweiler district. The exact date of its construction has not yet been clearly recorded. The first documented mention of the parish church dates back to 1166. At that time, the church is mentioned as the center of the parish at the time, which was located at the foot of Are Castle. In fact, these two listed buildings are closely linked. The then Count Theoderich I built Are Castle around 1100, only to commission the construction of the parish church located lower down shortly afterwards. The church, which is over 800 years old, has undergone many structural changes, so that part of the late Romanesque architectural style is now hidden or walled over.
