Of early medieval origin, it was rebuilt in 1284 by a Comacine master named Grazia. Of this phase only the north side and the lower part of the façade remain, with a facing made of large blocks of sandstone. The interior shows the structure acquired in the nineteenth century during a total renovation which also involved the raising of the building. However, the side altars and two sixteenth-century frescoes depicting the Crucifixion with Saints Rocco and Frediano, and the Madonna between Saints Jacopo, Sebastiano and Agata remain attesting to the ancient events of the interior furnishings. On the main altar is a painting depicting the Madonna and saints (1592) by Giovanni Maria Graziani from Bologna.
