The one-nave hall church originally built on this site from the second half of the 4th or the first half of the 5th century was already expanded by Archbishop Nicetas of Aquileia (454-485) into a large three-nave basilica. When the Longobards invaded Friuli in 568, Patriarch Paulus (557-569), who had since split from the Church in Rome due to the three-pronged dispute and had taken the title of Patriarch, fled from Aquileia to the Grado peninsula. Grado became the new seat of the patriarch. In the 6th century, Patriarch Elia (571-586) initiated the extension of the church building, which gave the church essentially its present form. After completion of the conversion work Elijah consecrated the Church 579 of St. Euphemia of Chalcedon.

Sant'Eufemia is the main church of the northern Italian city of Grado and former cathedral church of the Patriarch of Grado entitled the Basilica minor. The church is a rare, still largely intact example of a church from the time of the migration of peoples. In addition to church services, classical concerts are held in the church.
