The current structure, built in the 11th-12th centuries, incorporates Roman stonework. Originally named Galmanlith, it was renamed "Bootham Bar" in the 12th century due to nearby market stalls. The Bar features a portcullis, statues, and a large knocker installed in 1501. Notably, Scots seeking entry had to knock and obtain permission from city officials. In the 19th century, a section of the city wall connecting Bootham Bar to nearby fortifications was demolished. These fortifications protected St Mary's Abbey, once the richest in northern England. The abbey ruins are now part of the Museum Gardens. Today, Bootham Bar is a key stop on the City Walls Trail, offering historical markers and a rubbing trail map for exploration.

Bootham Bar replaces the porta principalis dextra of the legionary fortress, which lay immediately behind and beside the medieval gate. The Roman gate was probably still in use in the late 7th century, when it was mentioned as a boundary in a land grant. The alternative name of Galmanlith or Galmonelid, perhaps a scribal error for Galmouelid, refers to the gate of Galmou, the pre-Norman Conquest name for the hill on which St. Mary's Abbey was built.
