Today's location was not the original one. In the 14th and 15th century, numerous storm surges occurred. During this time probably the then center with church and cemetery was a victim of the floods. The church was probably built well before 1600 at the current location. The pulpit dates from 1603 and the baptismal font 1606. Two tombstones from the 16th century are located at the side of the church.

It is believed that the old church site in the Elbe was in front of the children's playground by the pool. The new church was built at the current location.
The church belongs to the so-called Schifferkirchen. The namesake of the church is the Blessed Virgin Mary, who is still the dominating figure in the altarpiece.
The introduction of the Reformation probably took place in Twielenfleth not long after Hollern (1540).
Due to dilapidation, the church was rebuilt in 1819 as a single-nave half-timbered building with brick infill on the old foundations.
The tower is entirely made of wood and has a roof of shingles.
The total height is 28m.
