The current building is an octagonal chapel from the first half of the 18th century, in Baroque style. It is made of brick, with limestone window and door frames and corner bands. It is covered by an octagonal mansard roof tapering towards the center. The building has been beautifully restored. It is surrounded by lime trees and, together with its surroundings, it was classified in 1980 as a protected monument or village view.

Legend
A woman from Velm had a child. This had died without having received baptism. The mother prayed to Our Lady, who revived the child. However, when there was a typhus epidemic in neighboring Montenaken, the inhabitants brought the statue of Mary there. But when the epidemic was over, they didn't want to bring it back. But the statue of Mary went back to Kerkom on its own occasion.

There is still a pilgrimage to this chapel on the Assumption of Mary (15 August).
