The old cathedral, officially the winter co-cathedral of the adjacent Santa Maria Assunta, is the co-cathedral of Brescia. Built from the 11th century on top of a previous basilica, it has undergone more than one extension over the centuries, but the original Romanesque structure remains intact. This makes it one of the most important examples of roundabouts in Italy, as well as one of the most important examples of Lombard Romanesque architecture.

The fact that this cathedral was built on top of a basilica can be seen by glass panels in the floor that look at the previous floor and foundation.

The building also contains numerous and important works, including a tomb of Bonino da Campione, the organ of Giangiacomo Antegnati, the marble sarcophagus of Berardo Maggi
