The Mutzbach rises in Odenthal-Voiswinkel at an altitude of around 175 m above sea level. NHN, crosses the Bergisch Gladbach districts of Paffrath and Hand and, after flowing through the Oberer Mutzbach nature reserve in the Dünnwalder Forest in Cologne, used to feed the Dünnwalder Waldbad. In the Manfort district of Leverkusen, it flows into the Dhünn at an altitude of 45 m above sea level, piped over the last few kilometers. NHN.

From a natural perspective, the Mutzbach rises in the Paffrath limestone depression, part of the Bergische plateaus. As it continues west of Paffrath, the Mutzbach reaches the Paffrath limestone terrace with a gravelly-sandy cover as part of the Bergische Heideterrasse. To the west is the Wahner Heideterrasse with gravel and sand deposits from the Rhine. In the subsequent Mülheim Rhine plain there is almost no gradient, so that the streams are not very pronounced. Flood clay deposits up to 2 m thick predominate over gravel and sand with brown earth and parabrown earth. The stream valleys have gley soils characterized by groundwater. There are floodplain soils in the area where it flows into the Dhünn.
