Located in Carezzano Maggiore, it is currently dedicated to Sant'Eusebio, but until 1988-89 it was also dedicated to Santa Maria Assunta: the double dedication derives from the transfer in 1576 of the parish of S. Eusebio di Carezzano Superiore to the oratory of S. Maria di Carezzano Maggiore, due to the dilapidated state of the former. In this way Carezzano Superiore lost its ecclesiastical district until 1648, when it was declared a parish again. In the 16th century the church of S. Maria Assunta and S. Eusebio had a small portico in front, which was demolished when the facade was restored. In 1947 it was embellished by the archpriest Don Ridella.
