This gate dates from the 14th century and was part of the city wall. It was first called "Porta Cittadina" after a captain of the Lambertesco Cittadini people. When the Medici built the fortifications in the 16th century (1565-1593), the gate was retained, but was renamed "Porta Reale". In 1755 a second gate was built in the north of the city, the "Porta Nuova". Demilitarization work (1848 - 1850) took place on the defensive wall and in 1854 the medieval watchtower that towered over the gate was demolished. Parts of the demolished defensive walls became public gardens or paths.

Porta Vecchia (translated: Old Gate) is the oldest city gate in the walls of Grosseto and is located at the southern corner of the fortifications. Together with the Cassero Senese, it is one of the two surviving architectural elements of the 14th-century city walls and survived the extensive reconstruction work carried out by the Medici in the late 16th century.
