In 1121, a chapel was first mentioned in Graach, said to have been dedicated to the apostles Simon and Judah. Based on indeterminate wall remains, the chapel's location was assumed to be near the Josephshof.

The Graach Church is a replica of the Kues Hospital Chapel, built between 1453 and 1455. Both buildings are in the Gothic style. Connections with the St. Cusanus Hospital had existed since 1532, when the Graach branch church, along with the mother church of Bernkastel, were affiliated to the St. Cusanus Hospital.

A church seal from 1785 reveals that the two apostles Simon and Judah are the patron saints of the Graach Church.
