Heimerzheim is located roughly in the middle of the triangle between Bonn, Euskirchen and Brühl. The immediate neighboring villages are Dünstekoven to the south, Neukirchen, Müggenhausen and Straßfeld to the west and Metternich to the north. To the east, the foothill villages follow about 8 km away.

The village lies on the western edge of the Ville on the northwestern edge of the Kottenforst. The Swist flows through the town in a largely artificially designed bed. To the west of the Swist there are very fertile soils in the Jülich-Zülpicher Börde, which are used intensively for agriculture. To the east, the landscape is primarily characterized by forest. The highest point is in the area of the Roman aqueduct in Kottenforst, the lowest is near Kriegshoven Castle.

The Swist jump is remarkable. It is a geological fault that runs along the slope through the town. The Swistsprung forms the border between two floes in the Lower Rhine Bay. Subsidence takes place along this fault, which is significantly accelerated by lignite mining and the associated swamping measures in the Rhenish lignite mining area. The Swistsprung is the cause of numerous structural damage and undevelopable properties in Heimerzheim.
