The environment is a single room, rectangular in shape; it was richly frescoed on the walls, on the vault and in the apse by the painter from Urbino Carlo Roncalli
The sacristy and the chapel of the crib open up on the corridor, before joining the entrance stairway. The latter, the only surviving room of the ancient building, constitutes as a whole a valuable and unique work in the world: the Urbino sculptor and decorator Federico Brandani, between 1545 and 1550, recreated the environment of the cave in which Jesus was born entirely covering the walls in tuff and pumice stone. In the ceiling the stuccoes form an animated angelic glory, while on the bottom there is the real nativity of Jesus Christ: under a hut is the holy family with the donkey, the ox and four shepherds in adoration. All the figures are in life-size stucco, with very realistic poses and expressions. The nativity scene is considered one of the sculptural masterpieces of Italian Mannerism.
